### Chapter Overview

Human reproduction involves the formation of gametes (gametogenesis), transfer of sperms (insemination), fusion of male and female gametes (fertilization), formation and development of blastocyst (implantation), pregnancy (gestation), and delivery of the baby (parturition).


### 1. The Male Reproductive System (рдирд░ рдЬрдирди рддрдВрддреНрд░)

Consists of a pair of testes, accessory ducts, accessory glands, and external genitalia.

  • Testes (рд╡реГрд╖рдг): Located outside the abdominal cavity in a pouch called the Scrotum (рд╡реГрд╖рдгрдХреЛрд╢), which maintains a low temperature (2тАУ2.5┬░C lower than normal internal body temperature) necessary for spermatogenesis.
  • Seminiferous Tubules (рд╢реБрдХреНрд░рдЬрдирдХ рдирд▓рд┐рдХрд╛рдПрдБ): Each testis has 250 compartments called testicular lobules, containing 1-3 seminiferous tubules.
    • Spermatogonia (Male germ cells): Undergo meiosis to produce sperms.
    • Sertoli cells (рд╕рд░реНрдЯреЛрд▓реА рдХреЛрд╢рд┐рдХрд╛рдПрдБ): Provide nutrition to the germ cells.
  • Interstitial cells / Leydig cells (рд▓реЗрдбрд┐рдЧ рдХреЛрд╢рд┐рдХрд╛рдПрдБ): Present in intertubular spaces; synthesize and secrete male hormones called androgens (testosterone).
  • Accessory Ducts: Rete testis $\rightarrow$ Vasa efferentia $\rightarrow$ Epididymis $\rightarrow$ Vas deferens.
  • Accessory Glands: Seminal vesicles, Prostate gland, and Bulbourethral glands (Cowper's glands). Their secretions constitute the Seminal plasma (рд╡реАрд░реНрдп рдкреНрд▓рд╛рдЬреНрдорд╛), rich in fructose, calcium, and enzymes.

### 2. The Female Reproductive System (рдорд╛рджрд╛ рдЬрдирди рддрдВрддреНрд░)

Consists of a pair of ovaries, oviducts, uterus, cervix, vagina, and external genitalia.

  • Ovaries (рдЕрдВрдбрд╛рд╢рдп): Primary female sex organs that produce female gamete (ovum) and several steroid hormones (ovarian hormones).
  • Oviducts / Fallopian Tubes (рдЕрдВрдбрд╡рд╛рд╣рд┐рдиреА): Parts include Infundibulum (funnel-shaped with finger-like projections called fimbriae), Ampulla, and Isthmus. Site of fertilization is the ampullary-isthmic junction.
  • Uterus (рдЧрд░реНрднрд╛рд╢рдп): Also called womb. Its wall has three layers:
    1. Perimetrium: Outer thin layer.
    2. Myometrium: Middle thick layer of smooth muscle (exhibits strong contraction during delivery).
    3. Endometrium: Inner glandular layer (undergoes cyclical changes during menstrual cycle).
  • External Genitalia (Vulva): Mons pubis, labia majora, labia minora, hymen, and clitoris.

### 3. Gametogenesis (рдпреБрдЧреНрдордХрдЬрдирди)

The process of formation of gametes in the gonads.

A. Spermatogenesis (рд╢реБрдХреНрд░рдЬрдирди)

  • Process: Spermatogonia ($2n$) $\rightarrow$ Mitosis $\rightarrow$ Primary Spermatocyte ($2n$) $\rightarrow$ Meiosis I $\rightarrow$ Secondary Spermatocytes ($n$) $\rightarrow$ Meiosis II $\rightarrow$ Spermatids ($n$) $\rightarrow$ Spermiogenesis $\rightarrow$ Spermatozoa (Sperm).
  • Spermiation: The release of sperms from Sertoli cells into the lumen of seminiferous tubules.
  • Hormonal Control: Hypothalamus secretes GnRH $\rightarrow$ stimulates anterior pituitary to release LH (acts on Leydig cells to secrete androgens) and FSH (acts on Sertoli cells to help in spermiogenesis).

B. Oogenesis (рдЕрдВрдбрд╛ рдЬрдирди)

  • Process: Oogonia ($2n$) formed during embryonic stage $\rightarrow$ Primary Oocyte ($2n$) arrested in Prophase-I $\rightarrow$ Birth to Puberty $\rightarrow$ Primary oocyte + granulosa cells = Primary Follicle.
  • At puberty, primary follicle matures into Graafian Follicle.
  • Primary oocyte completes Meiosis I (unequal division) $\rightarrow$ Secondary Oocyte ($n$) + First Polar Body.
  • Secondary oocyte forms Ovum ($n$) only after fusion with a sperm (Meiosis II completion).

### 4. Menstrual Cycle (рдЖрд░реНрддрд╡ рдЪрдХреНрд░ / рдорд╛рд╕рд┐рдХ рдзрд░реНрдо)

The cyclic cycle of events starting from one menstruation till the next in non-primate mammals (primates: humans, apes, monkeys). Duration: ~28/29 days.

  1. Menstrual Phase (Days 1тАУ5): Breakdown of endometrial lining of the uterus if fertilization does not occur.
  2. Follicular / Proliferative Phase (Days 6тАУ14): Primary follicles grow into Graafian follicles; endometrium regenerates through proliferation. Secretion of gonadotropins (LH and FSH) and estrogens increases.
  3. Ovulatory Phase (Mid-cycle, ~14th day): Rapid secretion of LH (LH surge) induces rupture of Graafian follicle and release of ovum (Ovulation).
  4. Luteal / Secretory Phase (Days 15тАУ28): Remaining parts of Graafian follicle transform into Corpus Luteum (secretes large amounts of Progesterone, essential for maintaining endometrium).

### 5. Fertilization and Implantation (рдирд┐рд╖реЗрдЪрди рдФрд░ рдкреНрд░рддреНрдпрд╛рд░реЛрдкрдг)

  • Fertilization (рдирд┐рд╖реЗрдЪрди): Fusion of a sperm with an ovum. It takes place in the ampullary-isthmic junction of the fallopian tube.
    • Sperm induces changes in the zona pellucida layer of the ovum to block entry of additional sperms (polyspermy block).
  • Cleavage: Rapid mitotic division of the zygote as it moves down the oviduct to form 2, 4, 8, 16 daughter cells called blastomeres.
  • Morula: Embryo with 8тАУ16 blastomeres.
  • Blastocyst: Morula transforms into blastocyst. It has an outer layer called Trophoblast and an inner group of cells called Inner Cell Mass.
  • Implantation: Blastocyst gets embedded in the endometrium of the uterus, leading to pregnancy.

### 6. Pregnancy and Embryonic Development (рдЧрд░реНрднрд╛рд╡рд╕реНрдерд╛ рдФрд░ рднреНрд░реВрдг рд╡рд┐рдХрд╛рд╕)

  • Placenta (рдЕрдкрд░рд╛): Structural and functional unit between developing embryo (fetus) and maternal body. It supplies oxygen and nutrients and removes carbon dioxide/waste. It also acts as an endocrine tissue producing hCG, hPL, estrogens, progesterone, and relaxin (during late pregnancy).
  • Embryonic Development Milestones:
    • 1 Month: Heart is formed.
    • 2 Months: Limbs and digits develop.
    • 3 Months (First trimester): Most major organ systems formed (e.g., limbs, external genitalia).
    • 5 Months: First movement of fetus and appearance of hair on head.
    • 9 Months: Fully developed fetus, ready for delivery.

### 7. Parturition and Lactation (рдкреНрд░рд╕рд╡ рдФрд░ рджреБрдЧреНрдзрд╕реНрд░рд╛рд╡рдг)

  • Parturition (рдкреНрд░рд╕рд╡): The act of expelling the full-term fetus.
    • Triggered by neuroendocrine mechanism involving Fetal Ejection Reflex.
    • Oxytocin hormone causes stronger uterine contractions.
  • Lactation (рджреБрдЧреНрдзрд╕реНрд░рд╛рд╡рдг): The mammary glands produce milk towards the end of pregnancy.
    • Colostrum (рдХреЛрд▓рд╕реНрдЯреНрд░рдо / рдЦреАрд╕): Milk produced during the initial days of lactation; rich in antibodies (IgA) essential to develop resistance in newborn babies.
